Getting to know your child

Before your child’s first appointment, fill in this short parent form: who your child is, what worries you, and what you’re hoping for. It takes about ten minutes, and it means the appointment itself is spent on your child instead of on paperwork. Print it, or fill it in on paper and bring it with you — to our children’s therapy centre in Multan, or to an online consultation from anywhere in the world. There are no right or wrong answers here, so leave anything blank, or write “not sure”, whenever that’s the honest answer.

What happens at your first appointment?

Your first appointment starts with a free, unhurried conversation about what you have noticed at home. A therapist then spends around 45 to 60 minutes with your child, playing rather than testing, watching how they talk, understand, play, move and pay attention. Before you leave you are told what we saw and what we suggest next. No referral is needed.

That is the whole visit. What comes after it — goals, sessions and reviews — is set out in how therapy works here, step by step. This form covers the history part, which is the slowest part of any first meeting.

What should I bring?

Nothing here is compulsory — come with none of it and we will still manage. But each of these saves us guessing:

  • This form, filled in as far as you got.
  • Any reports you already have — a diagnosis letter, hearing or vision results, old therapy notes, a school report. A photo on your phone is fine.
  • Anything a teacher has said, if a teacher is the reason you got in touch.
  • A comfort toy or a snack. Familiar things help a child settle in a new room, and a settled child shows us far more.
  • Your questions, written down. They vanish from your head the moment you sit down — the questions parents ask us most is a good place to start.

Which parts of the form apply to my child?

Skipping is expected, not a problem. Use whichever line below sounds like you:

  • Short on time? Sections 1 and 2 on their own are plenty.
  • A baby or toddler? Skip section 6 — it is only for children already at school.
  • Already has a diagnosis? Skip sections 4 and 5 and write in your own words instead. If the diagnosis is recent, the first weeks after a diagnosis may help before we meet.
  • Not sure this is even a problem? Read the situations parents describe most often and see whether your child is in there.
  • Dreading the appointment? Most parents do. Keep a few little reminders for the hard days in your pocket, and remember nobody in that room is judging your parenting.

💛 Before you start, take a breath. Fill in only what you can. Please leave anything blank that hasn’t happened yet, doesn’t suit your child’s age, or you’re simply not sure about. FAQ

Questions parents ask before the first visit

The things parents ask us in the day or two before they come in.

Do I have to fill this form in before we meet?

No. It is entirely optional, and your appointment goes ahead exactly the same without it. Filling it in simply means the first few minutes go on your child rather than on questions we could have asked beforehand. If all you manage is your child’s name, their age and what is worrying you, that is genuinely enough to be useful.

Do I need a doctor’s referral or a diagnosis first?

No referral is needed, and you do not need a diagnosis, a report or the right medical words. Parents contact us directly, usually with a plain sentence like “my three-year-old only says a few words”. If your child would be better helped by a doctor or another specialist, we will tell you and help you take that step.

How long does the first appointment take?

Allow a little over an hour. The conversation about what you have noticed comes first and is free and unhurried, and the play-based assessment that follows usually takes 45 to 60 minutes. The centre is open Monday – Saturday, 10:00 AM – 7:00 PM, so call or WhatsApp +92 314 6040262 before you travel and we will have a therapist free when you arrive.

What if I cannot remember when my child first walked or talked?

Leave it blank, or write a rough guess — “around two, I think” is fine. Nothing on this sheet is scored and no answer is checked against a chart. The picture you give us in your own words matters far more than exact ages, and if your child is older and already walking and talking well, you can skip that section altogether.

Can I write my answers in Urdu?

Yes. Write in whichever language comes most easily — Urdu, Roman Urdu or English. “Mera bacha bolta nahi” tells us as much as a careful paragraph of English. Sessions and parent coaching run in English or Urdu, whichever is easier at home, so nothing is lost by writing the way you actually speak.

We are meeting online. Should I still fill it in?

Yes, and it helps even more. Fill it in on paper, then send a photo on WhatsApp before your video call so the therapist has already read it when your child appears on screen. Online consultations run from anywhere in the world, in English or Urdu, and the same form suits them.

Is anything I write here sent to you automatically?

No. There is nothing on this page to submit — it is a printable sheet you fill in by hand and bring with you, or photograph and send if you prefer. Nothing you write is stored on this website. Our privacy notice explains how we look after the information you do choose to share with us.
